Aligned with the Code of Ethical Conduct and the Title IX Sexual Harassment and Related Conduct Policy, GW Community Members who become aware of a situation involving discrimination, harassment, sexual harassment, including sexual assault, dating/domestic violence, and stalking, or related retaliation are responsible for promptly reporting the matter.

You can also submit a report without naming the person who has caused harm, or if you do not know their identity. If you choose to remain anonymous, please be aware that the Office of Equal Opportunity will not be able to follow up with you regarding available resources and support options and the university may be limited in its ability to address the incident.

 

 

Title IX Incident Report Form

To report an incident of sexual harassment, dating or domestic violence, or stalking please complete this form. Title IX applies to students, faculty, staff, volunteers, visitors, vendors, alumni, independent contractors, individuals attempting to participate in university programs and activities, and others.

Discrimination, Harassment, & Bias Report Form

This form should be used to report allegations of discrimination and harassment based on protected characteristics, as well as related retaliation. Additionally, report incidents of bias such as antisemitism, islamophobia, racism, sexism, ableism, ageism (not an all-inclusive list) may be submitted using this form.
